Job descriptionBROOKSIDE MULTICARE NURSING CENTERDEPARTMENT: Adult Day Health Care ProgramJOB TITLE: Registered Nurse CoordinatorREPORTS TO: DirectorSUPERVISES: Registered Nurse, Recreation Therapist, Program Assistants, SocialWorkerCOORDINATES WITH: Food Service, Environmental Services, Plant OperationsEDUCATION/EXPERIENCE/QUALIFICATIONS:

• Assists in the nutritional aspects of the program.
• Assists in the development, supervision and implementation of the registrant’s care plan.
• Minimum of 5 years work experience including practice, teaching and supervision
• Provides a nursing evaluation of each registrant within 30 days of admission.Approved by: Effective Date:Job Type: Part-timeSalary: Up to $48.00 per hourSchedule: 8 hour shiftMonday to Friday Education: Associate (Preferred) Experience: Nursing: 1 year (Preferred) License/Certification: BLS Certification (Preferred)RN (Preferred) Work Location: In person
• Provides direct supervision for registrant care, and actively engages in care when necessary.
• Provide health education for staff, registrants and families.
• Familiarity with Long Term Care policiesPHYSICAL/MENTAL QUALIFICATIONS: No hearing or sight impairment that would render the person unable to function in their position.SCOPE: Responsible for promoting the delivery and coordination of comprehensive care to participants, through the activity of assessment planning and providing direct nursing care in accordance with the policies and standards of Adult Day Care.DUTIES AND RESPONSIBILITIES:
• Current New York State Licensure, B.S. preferred
• Demonstrates leadership abilities
• Reviews regiment of registrant at regular intervals with Program Director.
• Identifies health needs and provides for appropriate care.
• Assists in all program activities as needed.
• Keeps private physician and Director informed of registrant’s status.
• Knowledge of gerontology, psychiatric and restorative nursing
