Job Description

Work Hours: Full time (40 hours)

Includes some weekends or additional hours as assigned.

Duties:

  • Assist the activity Director and Assistant Activity Director with the MDS process including gathering data, establishing specific goals, developing an activity care plan, and completing electronic documentation. Will complete care conference reports and attend care conference meetings as assigned.
  • Assist the Director in the daily scheduling and recordkeeping of the Adult Day Program participants within the Activity Program, once the interview process and the care plan has been established by the Director.
  • Must be comfortable speaking in front of the department and groups and leading groups and have effective communication and reporting skills.
  • Assist the Director in scheduling, training, assigning and keeping volunteer records for the department as required.
  • Must demonstrate flexibility and be able to adapt to changes in procedures and sudden changes in assignments as needed.

Qualifications:

  • Must be CNA certified in ND or able and willing to become certified
  • Previous work experience in an Activity Program in a Long Term Care setting preferred.
  • APND membership required or obtained within an agreed amount of time, with continual education to maintain membership.
