The Health Coach for the Falls Prevention Program works closely with members and their caregivers to promote habits to improve quality of life and attainment of health goals using a Whole Person Care approach. The Heath Coach creates a customized plan to meet the clients? key risks through collaborative approach with the clients and their caregivers. The Health Coach engages the member identified to have high fall risk via telephonic channels as well as video calls, typically over a 9-month period.
Whole Person Care supports individuals as they build confidence to improve their coping skills, physical and emotional health, and take part in a healthy lifestyle.
The Health Coach is responsible for developing a health coaching relationship with members and assisting the member through the process of actively working towards better health by providing support, encouragement, and education on chronic conditions. The Health Coach supports coordination of care and condition management activities with members, caregivers, and providers.
Essential Functions:
� Develop a trusting relationship with members to support the member through the process of changing behavior and improving management of their health by utilizing motivational interviewing techniques and change management concepts.
� Complete a detailed home safety assessment to identify fall risk.
� Assist in coordinating members? relationships with multiple service systems. This may include activities such as:
� Present health education in a culturally appropriate format. For example, teaching a member how to prepare traditional foods using less fat
� Provide social support to improve members? adherence to medical treatment plan
� Encourage the member in adoption of habits that are conducive to improved management of their chronic condition
� Develop an individualized plan including goals and plan of action
� Serve as a resource to Care Management team members
� Contact and engage member?s providers of care / primary care giver
� May conduct telephone and video visits with members to support member engagement, care plan goals and outcomes.
� May perform outreach activities to assist in locating members and engaging in Care Management services.
� Utilize the Sagility Health computer system to maintain confidential and accurate records of member in accordance with Sagility policy and HIPAA regulations.
� Maintain current and accurate database of community resource contacts for outreach services and assist Care Management program staff in targeted communications activities.
� Other duties, as assigned.
Job Requirements:
Education:
� College degree in a health-related field; Occupational Therapy, LPN/LVN acceptable
� Current Health Coach certification from an accredited professional health coach training program in health promotion, chronic condition management, or health education (for example, Chronic Care Professional Certification, NSHC CHC) preferred
(Note: Certification may be required per client contract)
Experience:
� Minimum 2 years? experience as a Health Coach for chronic condition, disease management, or health education activities
� Experience utilizing Motivational Interviewing techniques and behavioural change theory to facilitate member adoption of positive changes and improve health
Functional/Technical Knowledge, Skills and Abilities Required:
� Strong member advocate: willing to go above and beyond normal responsibilities to provide the best service possible
� Ability to assist member in navigating the healthcare system and community-based resources
� Culturally sensitive and competent for assigned membership; may be required to reside in, and share experience, language, and culture of the community to be served
� Field work (member and provider facing) requirements:
- Current CPR certification
- Current PPD
- Current valid driver?s license and reliable transportation with automobile liability insurance coverage
� Excellent communication skills; oral and written
� Strong organizational skills; ability to multi-task and be nimble
� Ability to work remotely
� Proficient computer skills
