Health And Wellness Coach With Telephonic Health Coaching

POSITION SUMMARY:
The Health and Wellness Coach will provide wellness coaching during an FHCP supported programs for members who have chosen to participate in wellness program; including but not limited to health fairs, health assessments, and the FHCP rewards program. The coach will also connect via the fitFHCP Telephonic Coaching Program with members interested in pursuing lifestyle and health status changes. Health coach will display diplomacy, tact, and a positive approach to engaging members in lifestyle changes. This professional will always present a caring image while maintaining patient confidentiality. The Health and Wellness Coach will have a working knowledge of health, wellness, fitness and nutrition.
QUALIFICATIONS:

Education, Skills and Experience:

• Self-starter with excellent time management skills.
• Certification in Health Coaching, Wellness Coaching or Bachelors’ Degree in Health Education, athletic training, or nutrition required
• Reliable transportation with ability to travel to various facilities as needed.
• Clear English communications skills, written and oral. Spanish communication skills a bonus.
• Strong customer service skills.
• Possess a working knowledge of the Transtheoretical Model of Behavioral Change for Coaching or other evidence-based coaching models.
• Reliable internet access at home and intermediate computer skills. Use of Outlook required.
• Current Professional Rescuer CPR certification (only acceptable programs: American Heart Association and the American Red Cross) or ability to obtain certification with 90 days of employment.
