Job Description
Department: Transportation - Crossroads Care Center - Milwaukee is seeking to hire a part time transportation driver.
General Summary: Under the supervision of the Administrator, you will provide safe, timely and courteous transportation of patients and/or pre-arranged parcel delivery. Maintain accurate records of all runs as required. Have a valid State of Wisconsin drivers license.
Primary Responsibilities and Duties:
  • Adheres to department customer service, service excellence, and performance standards.
  • Review daily agenda of pre-scheduled calls and plan the day itinerary accordingly. Will work closely with the Director of Medical Records to organize the daily appoinments
  • Demonstrates safe work practices and attitudes, follows safety rules, works to prevent unsafe conditions and behaviors, and participates in organizational and departmental safety programs.
  • Carry out other responsibilities as assigned.
  • Report any problems that may cause a delay in pick-up, immediately. to the Administrator.
  • Complete a daily check vehicle safety list daily, before driving the vehicle.
  • Transport patients according to schedule.
  • Maintain calm, pleasant, courteous attitude towards patients and outside health personnel.
  • Must maintain confidentiality of all employee, corporate and patient records and information in compliance with Corporate Compliance Policies and Procedures and all state and federal regulations and legislation.
  • Maintain designated vehicle in a clean manner inside and outside, clean daily at the end of shift.
  • Report to work on time each day well groomed, and in a clean, pressed uniform.
  • Exhibits excellent customer service skills and behaviors toward internal and external customers and co-workers.
  • Demonstrates personal commitment to the Organization’s core values through active involvement in the performance improvement process.
Knowledge, Skills And Abilities Required:
Minimum Education: High School diploma or equivalent. Ability to read maps and a good knowledge of Assigned Service area.
Contact with Others: Excellent ability to communicate with patients and health personnel.
Certification, Licensure, Registration: Must possess a valid Driver's License in state of Wisconsin and be insurable under the liability insurance guidelines and eligible for driving privileges based on current policy. Must possess the following:
  • Must safely put a patients in the vehicle and secure the patients during the process of raising them and securing them in the vehicle.
  • Must pass the drug screen
  • Required to know how to operate the wheelchair lift.
  • Operates and maintains company equipment in a safe and professional manner.
  • Must pass a criminal background check & driving history check
  • Valid Driver’s License and over the age of 19 with no more than 4 accrued points (insurance requirement)
Working Conditions:
  • Work environment:Works outdoors, in weather common to the region, on a regular basis. If driving a wheelchair van must be able to bend and lift patients in wheelchairs as a regular part of the job.
  • Equipment used: large vans, patient lifts and assistive devices and equipment.
  • Physical Effort: Physical demands including lifting (over 50 pounds), climbing, pushing, stooping, bending, kneeling and crouching.
