Job Description
Job Responsibilities


  • Maintains current records on each youth documenting contacts, service activity and other required information within COC best practice standards and guidelines; manages administrative duties associated with operating an office as needed. Maintains minimum standard of productivity per month as established by COC.
  • Provides intensive Wraparound care coordination to 10 children with complex emotional and behavioral needs and their families to ensure they receive all required formal and informal services from a continuum of services provided by a variety of public/private agencies, independent professionals and natural supports available to the families. Available 24 hours to support families during times of crisis.
  • Schedules and facilitates monthly Child and Family Team (CFT) meetings as defined by high fidelity Wraparound standards. Completes assessments with each youth/family assigned and completes assessments with youth/family on an ongoing basis to monitor youths' progress in care as outlined by agency standards. Intercedes and advocates on behalf of youth and families to ensure appropriate, timely and consistent services, including traveling to and participating in IEP meetings, etc.
  • In accordance with high fidelity Wraparound standards and COC best practice standards, develops a strengths-based needs assessment/family story and family vision for each youth assigned. Monitors individual youth and family progress, CFT member assignments, and ensures the appropriateness and effectiveness of services to address the needs of the youth and family; facilitates the CFT in exploring changes and revisions to the plan of care as needed.
  • Develops creative approaches to provide necessary services to youth and families; designs and assists in the implementation of individualized services when more traditional services are not available or not appropriate; procures and/or requisitions services for youth within the guidelines of the state procurement code and COC regulations and with the CFT approval
Minimum and Additional Requirements


A bachelor's degree and professional experience in human services or social services programs.


  • Considerable knowledge of the fundamental types of behavioral health disorders in children and adolescents.
  • Ability to express and present plans effectively both orally and in writing.
  • Ability to initiate and maintain effective relationships with youth, families and personnel from various agencies and external advocacy agencies.
  • Ability to operate a personal computer and use various computer software to perform job functions.
  • Considerable knowledge of family systems theory, interviewing techniques and team building and facilitation skills.


Preferred Qualifications


A bachelor's degree in a human service or social sciences related field and one year of experience with children with serious emotional or behavioral health challenges.
